Falls and Anticipatory Care (Southern Locality)

NHS Devon (the Commissioner) sought to commission a service for a Falls and Anticipatory Service in Southern Devon. This is a primary care based service focussing on the prevention of falls and the identification of those at the greatest risk falling to provide access evidence based interventions for those at high risk of falls. The contract term is for a period of 12 months commencing on 16 October 2023.
