Transforming Care Plans – opportunity to bid in partnership with Dorset County Council NHS England South region have prioritised capital funding to support Transforming Care Plan delivery in 2017-18 and 19, with annual funding of at least £5m available in the South. The funding is available to support the discharge of inpatients and is anticipated to be predominantly used for the acquisition and adaption of homes for people with learning disabilities. The Council is keen to consider partnering to bid for funding. It is likely that partners will be developing and completing homes over the next 12 to 18 months and working in partnership with Dorset County Council, can provide these homes for people with learning disabilities. For 2018 we would like to develop multiple TCP funding bids from a number of housing providers to create a pipeline of 20 to 30 homes across the County. We are therefore seeking partners who will be building homes and could commit to providing between 5 and 10 homes each. Of particular interest, would be the Dorchester – Weymouth area, Wareham and Swanage area and Bridport. Following EOI, bidders will be issued with an evaluation questionnaire for completion. The date for submission of the questionnaire will be Monday 11 December 17.
