This service is to deliver a residential short breaks service for adults with learning disabilities, complex needs and associated conditions, their carers and families and to strive to achieve the outcomes set out in the individuals care and support plan. Lot 1: The primary outcome of this service is to deliver a residential short breaks service for adults with learning disabilities, complex needs and associated conditions, their carers and families and to strive to achieve the outcomes set out in the individuals care and support plan. This service is for adults with a diagnosed learning disability with a varying complexity of support needs, which may include health needs. The Service Specification for this Service sets out the requirements relating to adults with diagnosed learning disability aged 18 years until age 65 who may also have physical or neurological conditions; acquired brain injury; enduring mental ill health or life limiting conditions.
