Specialist Short Stay beds for the purposes of rehabilitation

Tender description

Rehabilitative support in relation to individuals physical health conditions (including stays for those with long term conditions) in a residential care environment, thereby also giving their carers a break, and as step-down from people who are being discharged from hospital. The location must be within the administrative boundaries of Somerset Council and have a wheelchair accessible route to a town centre of no more than 1.5 miles in order to support people to independently access local amenities as part of their rehabilitation. Lot 1: The length of stay in these block beds will vary by individual needs and circumstances, but will aways be time limited and someone would only be referred if they required the specialised rehabilitative support that they provide
