Tender description

The Community Rehabilitation Service provides holistic, trauma‑informed rehabilitation for adults in Bristol with complex and enduring mental health needs, primarily psychosis. Its purpose is to build independence, reduce relapse, minimise hospital admissions and out‑of‑area placements, and support people to achieve stable, meaningful lives in the community. It delivers two core functions: 24/7 residential rehabilitation at Wellbridge House, offering psychosocial interventions, medication management, physical health support and daily living skills development; and community rehabilitation, supporting individuals in their own homes or supported accommodation through structured rehab plans, proactive case management, crisis planning, safeguarding and multi‑agency coordination Lot 1: NHS Bristol, South Gloucestershire and North Somerset ICB are using this notice as an intention to award notice. Direct Award C has been used for the award of this contract. The existing provider is satisfying the original contract and will likely satisfy the proposed new contract. The services provided are the service specification are not changing considerably. NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board (ICB) and Gloucestershire ICB are currently working together in a formal "clustering" arrangement to prepare for their merger into a single, new organisation (name to be confirmed), effective from 1st April 2027.
