The provision of an Integrated Care Programme that provides person centred support for people aged 50+ in East Lancashire Lot 1: Integrated Care Programme- The service provides person centred support for people aged 50+ in East Lancashire who are at risk of an unplanned hospital admission, a high frailty score or any number of long-term conditions. Start Date: 01/04/2025 End Date: 31/03/2027 The approximate lifetime value of the contract is £500,688 however the final contract value is still to be finalised. The ICB are intending to award a contract to an existing provider following direct award process C.
