Tender description

RVS provide support for Pathway 0 patients to enable safe discharge from hospital. They also prevent readmission and ensure future avoidance of admission. RVS provide practical support for up to 12 weeks using predominantly volunteers to deliver the service. An extension of the existing contract is required to support with Winter Pressures. The RVS service is seen as a critical contributing factor to the success of the D2RA project, with resource currently underutilised in the team and the potential to base RVS Co-ordinators within hospital discharge teams at the three hospitals in Liverpool. This extension would also allow us to look at a longer term commissioning plan for the service
