Tender description

Individual patient placements commissioned by NHS Cornwall and Isles of Scilly Integrated Care Board in organisations providing out of county care home services, these can be in residential homes, nursing homes or other specialist site settings.<br/><br/>The commissioner has placed with the provider following an assessment of needs and the provider can continue to meet those specific needs Lot 1: These providers are one of a number of out of county providers who support specific specialist Packages of Care for Care Home clients. <br/>As there is a number of these providers, we are awarding under Direct Award B as we do not restrict access to the provider OR the patient. This is the only care home that can meet needs of the individual within an acceptable distance from family<br/><br/>Contract term: 27.08.2024 - 31.03.2026<br/>Possible extension period 12 months therefore 31.03.2027<br/><br/>Lifetime value: Activity based per individual package of care. Estimated value £200,000<br/><br/>This is an new service with an existing provider. Additional information: This is a Provider Selection Regime (PSR) confirmation of contract award notice. This contract has been awarded under the Health Care Services (Provider Selection Regime) Regulations 2023.
