Royal Volunjtary Service continuation of Provision

Tender description

RVS provide support for Pathway 0 patients to enable safe discharge from hospital. They also prevent readmission and ensure future avoidance of admission. RVS provide practical support for up to 12 weeks using predominantly volunteers to deliver the service. An extension of the existing contract is required to support the decommissioning process for this contract. The Home from Hospital Service ends on the 30th June 2025, however this extension is to support RVS through their redundancy period and further allows us to close the current service in a safe way and ensure affective support for service users.
