This service sets out the core requirements for the delivery of Children’s Home-based Care (CHBC) arrangements in the London Borough of Hounslow. It describes the key features of the service being purchased and required of suppliers. CHBC refers to a range of support services provided to children, young people and their families within their own homes. CHBC is designed to address the specific needs of children and young people with disabilities. The service aims are to enhance their quality of life, promote their wellbeing and enable them to remain within their family environment. The Supplier will support children/young people’s activities of daily living. CHBC is for those at risk of or to unable to access mainstream activities (such as leisure centres, youth clubs, sporting activities etc) due to their disability or whose care is such that it is necessary to the Authority to provide additional support to Parent/Guardian CHBC is for children and young people with disabilities. CHBC aims to promote inclusion, independence, and the wellbeing of children and young people with disabilities within their family and community settings.
