Tender description
RVS provide support for Pathway 0 patients to enable safe discharge from hospital. They also prevent readmission and ensure future avoidance of admission. RVS provide practical support for up to 12 weeks using predominantly volunteers to deliver the service. An extension of the existing contract is required to support the decommissioning process for this contract. Performance figures evidence that the service is underutilized and this three month extension will allow us to close the current service in a safe way. During this period, we will be looking at the Home From Hospital Service as a whole and commissioning plans for the future.
