Dementia Support Services, contract will deliver specialist information, advice, guidance and support with, and in, local communities across the Essex County Council administrative area, working with health and social care service providers in a way that supports person-centred care, sustainability and better outcomes for people living with dementia, their carers and families, thus ensuring that people living with and affected by dementia receive the right support at the right time in the right place.
