Extra Care Housing Care & Support Services - London Borough of Sutton

Tender description

London Borough of Sutton is re-tendering for a new contract under a block contract arrangement for the delivery of extra care housing care and support services to three designated extra care housing schemes in Sutton. The provision of extra care housing care and support services have been divided into the following Lots comprising of a specific number of flats and of a specific number of care hours per scheme for people eligible for services: Lot 1 - Belsize Court Lot 2 - Cloverdale Court Lot 3 - Dymond House Lot 1: Lot 1 - Belsize Court (i) 40 one bed flats for people with physical disabilities, dementia, sensory impairment and/or other long term conditions over 55 years of age (ii) 13 two bed flats for people with physical disabilities, dementia, sensory impairment and/or other long term conditions over 55 years of age Lot 2: Lot 2 - Cloverdale Court (i) 41 one bed flats for people with physical disabilities, dementia, sensory impairment and/or other long term conditions over 55 years of age Lot 3: Lot 3 - Dymond House (i) 26 one bed flats for people with physical disabilities, dementia, sensory impairment and/or other long term conditions over 55 years of age (ii) 10 one bed flats for people with support reasons related to mental health/learning disability under 54 years of age (iii) 9 one bed short stay assessment flats for all age groups (iv) 1 two bed planned respite flat for all age groups
