This service is aimed at reducing Urgent and Emergency Care service use in identified individuals through holistic case management and support. This is not a crisis response service, but will work on coordinating the actions of providers across the entire health and social care system to ensure that patient’s needs are met.<br/>Barca Outreach Support Team (also known as High Intensity Service User Service) Lot 1: The Contract Authority, NHS West Yorkshire Integrated Care Board intends to award a contract to an existing provider following Direct Award Process C.
