Beds to support the transfer of patients from acute hospital sites to community beds for assessment purposes Lot 1: Implementation of D2RA Bed Pathway service will support patients who are able to be stepped down from an acute hospital bed (i.e.: at POW Hospital) within 48 hours of being declared clinically optimised for discharge and have their ongoing care needs assessed in the D2RA bed.
