Cheshire Community and Palliative Care Services

Tender description

Cheshire and Merseyside ICB has directly awarded contracts via a contract modification for Cheshire community and palliative care services to the incumbent provider. Community Services for East Cheshire only, commenced on the 1st October 2023 for 12 months with the option to extend for a further 12 months. Palliative Care Services for Cheshire East and Cheshire West commenced on the 1st October 2023 for 18 months to 31st March 2025. Lot 1: Palliative Care Services in Cheshire East and Cheshire West The Palliative Care in Partnership (PCIP) service is a Cheshire-wide collaboration which provides home (domiciliary) care to people in Cheshire who have palliative care needs and who are receiving end of life care. The service supports patients and those important to them in their own home, to ensure that patients care needs are met to maximise quality of life during their final weeks. Lot 2: Community Services for Cheshire East The Community Service delivers local, accessible services such as district nursing, therapies, phlebotomy, podiatry, treatment room care. The Community Services contract for Cheshire East was previously put out for tender and then recommissioned in 2016 on the basis of the 'most capable provider'. There was a 2 year extension option included in the contract which was enacted in 2021 and expired at the end of September 2023
