Provision of a Healthy Communities Service for NHS South West London Integrated Care Board. The Healthy Communities Service is being established to support and strengthen active communities by increasing self-care, reducing social isolation and promoting independence. It forms part of the HCT programme and comprises three activities: 1. Local Community Partnerships (LCP) development; 2. Inclusion and engagement for insights from seldom heard communities (through a Community Builder); and 3. VCFS upskilling and support training programme. The anticipated, maximum total contract value (including any potential extensions) is £179,550 (2 years) with an annual value estimated at £89,775 per annum plus any locally agreed uplift.
