Tender description

The service aims to provide all children & young people (up to 25yrs) who have a learning disability, autism or both, who are at risk of admission to a mental health inpatient unit, or those already in inpatient units, with a named keyworker who will enable and empower the children young people and their families to navigate the system around them. Keyworkers will have sufficient authority and expertise to support the CYP and their family and act as a liaison point for them. The service aims to keep CYP from being admitted to hospital (where appropriate) and look to meet needs in the community. Where there is a hospital admission, keyworkers will, in collaboration with children, young people and their families and the Multi-Disciplinary Team, work towards an appropriate discharge.
