Tender description

Welsh Government funding has been made available to support the delivery of the ‘All Wales Dementia Care Pathway of Standards’. This funding is allocated to enhance the Dementia Connector role, a named contact who provides support and advice to people and their carers living with dementia from diagnosis to end of life.Allocated funding is being apportioned to 3 potential providers, to giving third sector organisations the opportunity to submit proposals for the allocated funding with ideas of how their organisations can contribute to the Dementia Pathway of Standards connected to the Dementia Connector workstream. The principles of the Dementia Connector role are defined below:
