Local award to an East and North Hertfordshire (ENH) Primary Medical Care Services Provider via the Most Suitable Provider process. Primary Care led, in collaboration with local system partners, to deliver an integrated Enhanced Health service in all care homes (residential and nursing), as registered by the Care Quality Commission (CQC), supported by East and North Practices, and which are covered under the PCN DES/DES plus. Inclusive of non-weight bearing and Discharge to Assess beds (DTA) as well as Learning disability adult care homes. Exclusive of short stay respite services, neurological services, and flexi care schemes. Exclusive of short stay respite services, neurological services, and flexi care schemes. <br/><br/>The aims of the service include: <br/>• Improvement in quality of life for residents living in care homes. <br/>• Increase and improve implementation of Advanced Care plans (ACP) including Recommended Summary Plan for Emergency Care and Treatment (RESPECT) and Treatment escalation plan (TEP). <br/>• People to be cared for in their preferred place of care (PPC) and to die in their preferred place (PPD)<br/>• Partnership model delivery centred on primary medical care working collaboratively with all system partners to avoid duplication.<br/>• Ensure ICB pathways are implemented and utilised. <br/>• Identify best practices and the need for care home staff training and signposting to the relevant services. <br/>• Identifying interventions needed such as nutrition, hydration, oral health care and wider primary care services including structured medication reviews and opportunities for reduction in the medication where appropriate.<br/>• Efficient Data sharing and monitoring using the appropriate SNOMED codes. <br/>• Shared care records updated in order that the resident’s record is visible to system partners for clinical care. <br/>• Reduce avoidable admissions to secondary care.<br/><br/>This contract is a 2 year pilot with an option to extend for a further 1 year at the discretion of the commissioner and the spend is £822,000 per year.
