Integrated Community Rehabilitation Service

Tender description

To procure an integrated community mental health rehabilitation service, with a minimum of 14 units, which will form an integrated partnership between clinicians and the voluntary sector and offer specialist rehabilitation intervention. The Service will enable people to receive a community-based offer within South West London and avoid admission into a rehabilitation unit. It will also strengthen the level of care and support for service users requiring rehabilitation after their discharge from the hospital. Lot 1: Description of the services to be provided: The Service will provide specialist-enhanced rehabilitation support to adults typically aged 18-65 diagnosed with severe and enduring mental health needs who would benefit from an intensive rehabilitation and recovery-focused approach, which will best be met by an integrated clinical and voluntary sector shared model of care and support within an accommodation-based service. The clinical element will take the form of in-reach, typically including psychiatry, occupational therapy, nursing, and psychology input. This will be provided by the local Mental Health Trust Community Rehabilitation Team who will in-reach to a voluntary sector provider-led 24 hour supported accommodation service Lifetime value of the contract is : To note the maximum estimated total contract value for the SWLSTG service VCS service provision agreed in the business case (subject to NHS inflation) is £697,720 per annum, and £3,488,600‬ over the total contract length of 5 years. Dates between which the contract provides for the services to be provided: 01/02/2025-01/02/2028 + 2 year extention (contract start date may be earlier depending on mobilisation)
