Benefits support for Population Health & Care Programme

Tender description

The population health Programme uses the Oracle Population Health platform (HealtheIntent) which has been in place since 2020. HealtheIntent provides is an integrated data set comprising of four core data sources that refresh overnight to give almost real time patient identifiable data. There is also the ability to use the platform to upload other data sources as a snapshot in a semi-automated way as has been the case for the patient tracking list for surgery and adult social care data. The contract for HealtheIntent is up for review in March 2025 when it has one more year to run until March 2026. A decision will need to be made as to whether the contract is extended beyond March 2026 for a further year or if there is likely viable alternative to switch over from HealtheIntent to a London platform - assuming such an alternative is ready to use from March 2026. We are looking to have a final report ready for review by the Population Health and Care Board and other SE London stakeholders by March to inform the contract decision from April 2025.
