Supportive Technologies for Older Persons (STOP) @ Home for CHO 5 and IEHG

Tender description

The scope of the project aims to remotely monitor 95 participants residing in IEHG/CHO 5/SECH over an initial 12-month period. An independent academic partner will conduct an impartial project evaluation focusing on the Quality of Life of the Older Person and their families while also analysing data collected in relation to A and E/ acute avoidance, delayed long term care placement whilst ensuring VFM in compliance with National Financial Regulations. This initiative of CHO5 / SECH and the Ireland East Hospital Group in alliance with a supportive technology partner will support the older population continue to live at home as they choose supported by the introduction of supportive technology in their home. The purpose of the project takes into consideration what matters most to the older person in relation to domains such as personal health goals, safe mobility and function and medication support whilst taking into account cognition and mental health status.
