WP3958 - Discharge to Assess Pathway 3 (Complex Nursing Care)

Tender description

The Council wishes to appoint providers to support hospital discharge for people who require a care home setting. For people who require bed-based 24-hour care: includes people discharged to a care home for the first time. Those discharged to a care home for the first time will have such complex needs that they are likely to require 24-hour bedded care on an ongoing basis following an assessment of their long-term care needs. This block contract is for a maximum of 5 x Pathway 3 (complex nursing care) beds per provider There will be a maximum of 3 nursing complex placements per home at any one time Additional information: Please note that Walsall Council has a new Contract opportunity.
