D/deaf Services Framework Agreement

Tender description

A single Provider Framework Agreement to deliver services to D/deaf people. The Framework Agreement will have a duration of four years. All services provided under this Framework Agreement will be commissioned to do some or all of the following: • Intervene at the earliest opportunity to support prevention by encouraging people to be more proactive about their health and wellbeing. • Increase independence and reduce or delay the need for care and support services, including navigation of prevention and community services and effective self-care. • Improve access to information to support self-care, independence and to promote health and well-being. • Work, including in partnership with other organisations, to address inequalities. • Use strengths-based approaches, drawing on the skills and assets across people and their communities, to create opportunity. • Activities that enable effective communication, including British Sign Language related services and interpreting Keywords: Deaf, deaf, Support, Independence, Prevention, Communciation
