WP3926 - Housing Support Service - to support adults with mild learning difficulties and mental health issues

Tender description

The Council wishes to procure a single organisation to deliver services for individuals aged between 18 and 65 years who have a mild learning difficulty and / or mental health issues whose housing tenancy is at risk. The purpose of the housing support service is to prevent the loss of tenancy and provide advice and support to prevent the need for funded health and social care. The service will support to prevent the loss of tenancy, uphold the rights of people who are threatened with homelessness, support people to understand the broad range of housing related issues including responsibilities under their tenancy agreement and to manage these independently.
