Tender description

South Yorkshire Integrated Care Board (SY ICB) is seeking to appoint a single provider for the provision of a Social Prescribing Service. The Service will enhance the health and wellbeing of the residents of Rotherham and help decrease health inequalities, by providing a liaison and support service for people at high risk of unplanned urgent and emergency care to access non-medical sources of support in the community. The Contract will be for a term of three years with the opportunity to extend for 2 years subject to mutual agreement between the successful Bidder and the Authority. The maximum term of Contract is consequently five years. The available budget is £709,661 per annum. This figure includes all aspects of the service to be delivered, any bidder including caveats to this service may be considered as submitting a non-compliant bid.
