Chelmsford West Integrated Polyclinic for Serious Mental Illness (SMI) for NHS Mid and South Essex ICB

Tender description

This is a Contract Award Notice to communicate a contract award made by the Contracting Authority; NHS Mid and South Essex Integrated Care Board, (hereinafter referred to as "the Authority") in compliance with their local standing financial instructions (SFIs). The contract award is made to Chelmer Medical Partnership for provision of a Polyclinic with an integrated multi-disciplinary team (MDT) for Serious Mental Illness (SMI) across Mid and South Essex. The Polyclinic will run weekly on a Saturday from 9am to 1pm from Tennyson House Surgery and seeks to deliver a full holistic offer to patients with SMI who not only fall into a "Plus 5" cohort, but also fall within the Chelmsford West "Core 20" cohort. The weekly Polyclinic will see the patient assessed by four core members of the MDT; a Health Care Assistant (HCA), General Practitioner (GP), Mental Health Practitioner (MHP) or Psychologist, and Health and Wellbeing Coach. The contract term is 12 months, commencing 1st April 2023 and expiring on 31st March 2024, at a total aggregate Contract Value of £111,892.36. The Authority is publishing this Contract Award Notice in the Contracts Finder in accordance with the Public Contracts Regulations 2015 (as amended by The Public Procurement (Amendment etc.) (EU Exit) Regulations 2020) for spend above £30,000.
