Tender description

The Staying Out Project enables everyone to live at home longer, be healthier and get the right support where required, whether this be provided by health and/ or social care. The service uses arts and cultural activity to strengthen its local community. It engages people aged 65+ who are residents of Stockton-on-Tees, and are leaving hospital, or recognised as being at high risk of readmission; as well as those identified as being socially isolated. Service users predominately identified as having chronic health conditions, including dementia, mental ill health, and Parkinson’s Disease, and mobility, cognitive or sensory impairments. The service is designed to help participants stay active, remain independent, decrease hospital readmissions and poor mental/physical health, and lead fulfilling lives through a 6 week free of charge intervention. It is a creative, ‘social prescribing’ alternative to day care, which uses weekly, artist-led creative arts activity to keep socially isolated older people active and signposts to other creative activities or other services in the area.
