Tender description

Funding has been received to provide support to the RUH and wider system to support people discharged from hospital or hospital avoidance. This project is being delivered by the Community Wellbeing Hub. The hospital packs provided 72 hours of food supplies alongside a follow up call day 1, day 7 and then a further call 28 days to ensure the patient has access to wider community services available. We have managed to purchase items from various sources to make up 200 packs. We have funding available for a further 200 packs and Parsley Box have confirmed that they can make up the packs in full for a fixed price. This saves us considerable resources in purchasing the food and packing. Keywords: Parsley Box
