Tender description
North Lincolnshire health and social care economy are working to ensure people are only admitted to hospital when there is a clear clinical need that cannot be met in the community. It also aims to discharge people as soon as they are medically fit. This is particularly important during winter periods when healthcare demand is higher. Timely discharge of people from hospital is important in ensuring smooth patient flow through A/E and to subsequent wards and supports the target of a maximum 4 hour wait from A/E arrival to admission. North Lincolnshire CCG is seeking to commission additional community capacity to support discharge of people from hospital who meet the criteria for health transport1; • Where the medical condition is such that they require the skills or support of PTS staff on / after the journey where it would be detrimental to the patient’s condition or recovery if they were to travel by other means • Where the patient’s medical condition impacts on their mobility to such an extent that they would be unable to travel by other means This service will provide additional capacity to the current Patient Transport Service provided by East Midlands Ambulance Service. Outcomes The transport service will aim to meet the following NHS Outcomes Framework Domains and Indicators; Domain 2: Enhancing quality of life for people with long-term conditions Domain 3: Helping people to recover from episodes of ill-health or following injury Domain 4: Ensuring people have a positive experience of care Domain 5: Treating and caring for people in a safe environment and protecting them from avoidable harm Service Delivery The Service will support and enable individuals who are assessed as medically fit to be discharged to safely return home from the Scunthorpe A&E Department and bedded wards and meet the criteria for health transport.
