Primary Care Networks (PCNs) were established as part of NHS England and Improvement (NHSEI) long term plan to build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home NHSEI is looking for a Supplier for the provision of facilitation of discussion and action learning sets to support Primary Care Networks (PCNs) to work with their diverse communities to ensure people feed into decisions about health services and health outcomes.
