Tender description

NHS North West London (NHS NW London) requires Provision of Severe Mental Illness (SMI) Health Checks Outreach services in the borough of Harrow starting December 2022 to June 2023. NHS North West London Integrated Care Board requires the provision of out-reach support services through the Voluntary and Community Sector in Harrow Borough. The aim of the service is to provide peer support in the community to services users accessing physical health checks. The service provider will be expected to work alongside a number of close partners who provide a range of health and social care services. Responsibilities include: -Raising awareness and promotion of the SMI annual physical health checks and their importance. -Communication: outreach to, and training for, organisations across Harrow who come into contact with SMI patients or their carers; and outreach to patients via a variety of channels. -Prepare flyers to be distributed to GP practices about the service. -Attendance at the GP Forum/ Masterclass to discuss with GPs how we can support them and work with SMI patients to access their annual physical health check. -Setting up a secure NHS email address so that GPs can make safe and secure direct referrals to us of patients from their SMI register who need to attend their health check and may need support to do so. -Developing an outreach arm of the project. -Employ and train additional outreach workers to engage with and support those service users who have particular support needs e.g. due to religious/cultural issues for a female worker or for someone who can speak their community language. -Acknowledge the role of lifestyle factors on physical health and share information and support patients to access support for; smoking cessation, reduction in alcohol consumption, and attendance for cancer screening. Support Primary Care with encouraging people with SMI to have their COVIC Booster Vaccinations and Flu Vaccinations. NHS NW London is committed to assess the needs of the service users and consider how (if relevant) it needs to secure the needs of the people who use the services, and improve the quality and efficiency in the provision of the services including through the services being provided in an integrated way. The service is required for a 6 month period starting in December 2022. Harrow based capable providers may come forward by 17:00 on 22nd November 2022 to engage with NHS NW London ICB.
