Transformation Roadmap

Tender description

The Authority require a time-limited piece of work that sets out a clear joined up multi-year transformation plan to enable them to effectively prioritise and target their collective resources and monitor progress towards delivering their vision. This will also form part of the submission of the Hillingdon Hospital Outline Business Case (OBC) in October 2021. The key outputs from this work include (but are not limited to): - An overarching vision and narrative that articulates and joins up their health and wellbeing priorities and new model of care with the transformation required for the hospital redevelopment. - A description of the future state of their model of care and the enabling digital, estates and workforce elements that will be critical to delivery. - A phased five-year delivery plan to 2026 with key milestones relating to delivery and activity shifts (to be modelled in partnership with redevelopment analytics team). - Identification of the data sources and architecture required to monitor impact and delivery. - Articulation of the resources required to deliver the programme and likely sources for it, including clinical leadership, across the partnership. - A QIA that identifies areas of impact on inequalities in Hillingdon and any mitigations required including ongoing patient and resident engagement.
