Tender description

1.1 Model - To provide an integrated housing support service for people aged 18 and over who are homeless or at risk of homelessness and in need of intensive, housing support. 1.2 Access - Referrals are to be made via Carmarthenshire County Council Housing Options team. 1.3 Accommodation - Support is not tied to any particular accommodation. Therefore, this service is a non-accommodation based service. 1.4 Location - The service will be provided across Carmarthenshire. 1.5 Client Group - People who are leaving prison with co-existing needs; People who are, or at risk of being homeless and have a history of offending, mental health or substance misuse issues. 1.6 Length of Support - There is no time limit to this service. The length of support will be determined by the needs of the person being supported as detailed in their plan of support. 1.7 Number of Hours - 172 hours of support expected to be delivered over 12 units. This service will provide an intensive package of floating support to people who are aged 18 and over and who are homeless, at risk of homelessness and who have multiple, co-occurring needs. The floating support will include supporting the person find suitable accommodation. The client group to be supported includes prison leavers who are at risk of homelessness. Supporting the Welsh Government’s prisoner pathway, this service will work with people leaving prison to source and secure permanent accommodation and support them to re-establish themselves in the community.
