Tender description

To provide residential and community short break services to children and young people with a disability. This could be during the daytime including after school, weekends school holidays and overnight community/residential short breaks, operating in both a planned manner and in response to crisis or emergency situations. The provider(s) will ensure a person centred, outcome based approach which will underpin service delivery. Customers will be actively involved in the development and review of their person-centred support plan and any other plans associated with their care such as risk management and behaviour management plans.
