We would like you/your organisation to completed the attached questionnaire and return to Shanta Joseph via the email address given by 2.00pm 30th September 2020. HPS is a preventative service that is focused on: • Reducing the risk of accidents and injuries at home • Reducing the number of unplanned hospital admissions of older people • Reducing the numbers of older people unnecessarily entering permanent care • Promoting independence. • Ensuring a sense of well-being, safety and security is achieved for people who use this service. The HPS delivers low-level interventions such as repairs and maintenance services, checks around the home (such as energy, fire and security checks), first-contact, referral services and other housing maintenance-related services to older individuals at a very low cost. It also helps to support unpaid carers to maintain their caring role. The overall aim of this service is to reduce risks, hazards and improve safety enabling improved well-being. The HIA enables Camden residents to continue to live independently by delivering the following service: • Deliver a professional Home Improvement function to meet the needs of Camden residents and the requirements defined in Housing Grants, Construction and Regeneration Act 1996 and the Care Act 2014 • Supports Camden through the Disabled Facilities Grant (DFG) process enabling eligible residents to obtain a DFG and project manage major adaption work. • Support the reduction of Delayed Transfers of Care (DToC) and improve the success of admission avoidance • Reduce the need for more costly care and allow people to remain independent in their own homes for longer • Assists people to maintain, repair and improve their homes to make them safe and suitable. Please see the attached questionnaire for more detail.
