Expanded and transformed provision of integrated diabetes specialist services to reduce the number of complications related to diabetes by moving focus to prevention, early identification and improved management approach. The existing contract includes within its scope for the provision of district general and associated acute and community sector health care services, including consultant led outpatient services, diagnostic services, elective day-case, surgical and in-patient services and district nursing. Recent service improvement and redesign has led to the development of an expanded and transformed provision of integrated diabetes specialist services to reduce the number of complications related to diabetes by moving focus to prevention, early identification and improved management approach. Development of existing consultant led outpatient diabetes services into a new integrated diabetes specialist service that aims to reduce the number of complications related to diabetes by moving focus to prevention, early identification and improved management approach. The additional investment will finance increased healthcare workforce capacity and capability to more deliver more efficient and effective healthcare outcomes for patients. The solution will enable implementation of an integrated care pathway that must necessarily be provided within the existing elective services of the NHS Trust. Additional investment is being made to the existing contract, through a contract variation procedure (contract modification) to enhance existing healthcare provision already being delivered by the provider, i.e. consultant led diabetes services, to deliver health outcome benefits to patients referred into this diabetes care pathway from primary care.
