Tender description

Vulnerable adults housing and well-being support services. The aim of this commissioning activity is to enable vulnerable adults to live healthy, happy independent lives within communities. The new contract will start from 1.12.2019 until 30.11.2022; with a potential contract extension of up to 2 years subject to satisfactory performance and funding availability. The services are designed to meet the commissioning model and collaborative approaches set out in the vision and strategy for adult social care in Birmingham, domestic abuse prevention strategy and the homelessness prevention strategy. The 3 strategies call for an approach that embeds early and personalised support and prevention, to reduce the impact of more costly crisis stage interventions which are based upon statutory thresholds or definitions. This includes the importance of making safeguarding personal. Provision of face to face information within an accessible centre providing, advice and guidance to vulnerable young people aged 16-24 living in precarious housing circumstances. Early targeted support in the home for vulnerable young adults aged 16-24 to avoid heading into crisis or worsening health and wellbeing. Provision of a safe place of shelter for vulnerable young adults aged 16-24 to expedite support for swift move-on into appropriate provision. Accommodation based intensive support that is trauma/psychologically informed for vulnerable young adults aged 16-24 to aid recovery and healing and support transition into independence. A supported housing solution that provides accommodation with support for vulnerable young adults aged 16-24 with multiple needs to build resilience, improve wellbeing and move towards independence. Provision of face to face information within an accessible centre providing, advice and guidance to vulnerable single adults and couples aged 25 plus living in precarious housing circumstances. Early targeted support in the home for vulnerable single adults and couples aged 25 plus to avoid heading into crisis or worsening health and wellbeing. Provision of a safe place of shelter for vulnerable single adults and couples aged 25 plus to expedite support for swift move-on into appropriate provision. Accommodation based intensive support that is trauma/psychologically informed for vulnerable single adults and couples aged 25 plus to aid recovery and healing and support transition into independence. A supported housing solution that provides accommodation with support for vulnerable single adults and couples aged 25 plus with multiple needs to build resilience, improve wellbeing and move towards independence. The service is provision of a Rough Sleepers Outreach Service to reduce levels of rough sleeping in Birmingham. The service will also work with the aim of improving the health and wellbeing of rough sleepers and minimising their risk of harm. Early targeted support in the home for vulnerable adults who are or have been victims of domestic abuse to avoid heading into crisis or worsening health and wellbeing. Provision of a safe place of shelter for victims of domestic abuse to enable rapid assessment of need, delivery of support and engagement of specialist agencies to start the process of healing and recovery and to expedite move-on into appropriate provision. Accommodation based intensive support that is trauma/psychologically informed for vulnerable single women to aid recovery and healing and support transition into independence. Provision of dispersed refuge accommodation comprised of individual tenancies within the community with the relevant safety and security features that provides support to build resilience, improve wellbeing and move towards independence. Early targeted support in the home for vulnerable families with multiple needs living in temporary accommodation or homeless at home to avoid heading into crisis or worsening health and wellbeing. Provision of face to face information within an accessible centre providing, advice and guidance to vulnerable adult offenders upon prison release with multiple needs living in precarious housing circumstances. Early targeted support in the home for vulnerable adults with an offending history plus to avoid heading into crisis or worsening health and wellbeing. Accommodation based intensive support that is trauma/psychologically informed for vulnerable adults with an offending history to aid recovery and healing and support transition into independence. A supported housing solution that provides accommodation with support for vulnerable adults with an offending history with multiple needs to build resilience, improve wellbeing and move towards independence.
