Tender description
Short-term 5 month programme to support NCL in improving finance and quality of CHC services in advance of CCG merger and aligned with NCL CCG financial recovery plans. Expert knowledge and experience of CareTrack system essential to enable business analytics and data management. End-to-end services for the following main areas CareTrack and continuing healthcare over 5 months, 7 days a week, ensuring compliance with MCA, DOLs and best interest, CHC national framework and all relevant legislation for: — domiciliary commissioned care data and management, — backlog/overdue reviews up to a maximum of 850, — prospective/upcoming reviews to 31.3.2020 up to a maximum of 750, — quality control of DSTs/assessments completed (1 month time frame) up to a maximum of 125. Provider must have a proven track record of delivering successful large scale CHC projects which improve the quality of patient care and deliver efficiencies. It is critical that the provider has the ability to support this with expert knowledge and experience of the CareTrack System (CTS). This includes having immediate and secure direct access to patient identifiable data and the ability to update in real-time, complete complex analysis of CTS data to ensure resources are most efficiently targeted to deliver project deliverables. The provider will provide CTS training to CCG staff to ensure that the CTS is used to its full potential and that best practice and consistency in CTS is embedded across NCL. The provider will deliver a fully functional end-to-end data and analytical management, including comprehensive project management, ensuring CTS data integrity and monitoring impact of all cost/efficiencies and real-time CTS updates for 3 and 12 months reviews. Clinical reviews are face to face and includes attending panel, working with key partners including Councils. Reviews will be priced on a unit cost per case on sliding scale (low to high) and on sign-off by CCG Lead. A clinical review to be undertaken where indicated, inclusive but not limited to: — analysis of the CHC patient data to undertake a triage of the existing patient cohort to ensure that any material change in patient need is identified proactively and actioned appropriately to ensure an efficient delivery, — all 3 and 12 month reviews, backlog and prospective for fully funded, fast track, FNC and joint funded cases and due by 31.3.2020, — will arrange and coordinate all review appointments with patients and their advocates at a suitable date and time and also with MDTs, — will gather essential information and evidence to support the review and DST if required, — a face to face review visit to compare/contrast any material change in need, — will review the cost of commissioned care in regard to need (best value consideration), — where there has been a material change in need, the provider will make a recommendation to the CCG and full MDT approach in line with the CHC national framework, — will ensure a quality assurance process for any MDT eligibility recommendation and keep the CCGs CHC database up to date for governance and reporting purposes, — will present the MDT recommendation to a joint CCG and adult social care panel if the recommendation is that the individual is no longer eligible, — liaise/work with local authority as appropriate, — ensure compliance with MCA, DOLs and best interest and all relevant legislation. The requirement includes the delivery of a domiciliary care management and monitoring service on CT, embedding best use CTS, intrinsically linked to the analytics and ensuring data accuracy through real-time update. A reduction in the CHC domiciliary care budget of a min. 5 % per annum cost saving is to be guaranteed through the invoice management. Financial non-achievement will be netted from the contract value. Service includes verifying commissioned care and updating the CTS to reflect actual care delivered, changes in packages and triggering a review if required. A 1-month NCL Quality Control service of DSTs to ensure compliance with framework and appropriate conversion within 48 hours of DST being completed by the CCG. This will include analysis of the NCL eligibility rates compared to national and cluster averages, to identify any outliers in CCGs and the steps necessary to address any issues. This includes feedback on the quality of assessment, providing training and support as required and working with the project team to embed improvements. The project management must have a named senior lead. The CCG will provide resource to ensure service quality and financial assurance. No operational or CTS support will be provided by CCGs. Based on the requirement of the service it is planned that CHS Healthcare are commissioned to deliver this service.
