Tender description

Commissioned services will be based on a 2-tier approach. The first tier will offer information advice and guidance to support customers to preventative services and encourage the adoption of healthier lifestyles. The second tier will include action planning for customers with more complex needs. Customers will be engaged in the development of their own action plans using a strength based, and where appropriate, Adverse Childhood Experiences (ACE) informed approach. The provision of a single point of access for all health and social care related services for people of all ages and for the provision of Information Advice and Guidance (IAG) to help people access those services. Outcomes will include a ‘step change’ in community and population understanding of available preventative services and opportunities and enhanced partnership working to improve service delivery, access to services and successful collaborative funding bids. The provision of support for older people regarding guidance, advice, befriending and action planning. The commission will achieve the following outcomes: — reduce the number of older people who suffer from social isolation/loneliness, — increase the number of residents who feel connected with their friends, family and community, — increase the number of residents who can live safe and well in their own homes — with healthy and happy life expectancy, keeping active with confidence and self-esteem, — increase the number of older people who know about and feel confident to access opportunities and services, to get out and about and contribute to their community, — improve partnership working to develop services and more streamlined pathways between services across the system and for peer support, — enhance community awareness of how to access preventative services, — explore and promote new ways for older people to maintain self-care, skills and abilities, — increase the number of successful collaborative bids for further investment, — increase the number of friends and family who have the knowledge and feel confident to support older people with their resilience, skills and abilities, — manage people’s needs in the community, unless there is an absolute medical/care need for them to be in hospital/residential care, — integrate support around the needs of the individual through a personalised approach to care, — promotes self-care and resilience by building and utilising community assets and engaging in the co-production of care. This commission aims to reduce the number of people (including young people) whose circumstances cause barriers to achieving their aspirations for work, training and education. The commissioned service will help to transform the lives for people who want to develop positive futures, but face challenges and barriers which they find too difficult to overcome without support. People supported will include those with learning and physical disabilities. As a result of this commission more people will feel ‘in control’. They will be more resilient to challenges and will feel more able to take advantage of opportunities. They will be able to define and sustain their own future with aims, a sense of direction and motivation. For the provision of services for adult carers within Blackburn with Darwen and those who care for someone who lives within the Blackburn with Darwen Borough Council boundary. This document will describe the service commissioned, how it should be delivered and how it will be monitored. The aim of the service is to deliver information, advice and guidance to young carers who live in Blackburn with Darwen, or (by arrangement) who care for someone who does: — to facilitate support to young carers who live in the Borough of Blackburn with Darwen, — to provide a more in depth service to those young carers with more complex needs, and those who face barriers to full participation in the assessment process, — to facilitate access to positive activities as a break from caring, — to work in partnership with health and social care organisations, and other agencies to provide this support, — to raise awareness of young carers’ needs and liaise with key professionals including GPs and schools, — to raise awareness of the signs and symptoms of issues affecting young carers to support early identification, in partnership with other early help services such as schools, — to ensure that the needs of young carers and their families are assessed at the appropriate level of the Continuum of Need and Response Model using the CAF process where applicable ensuring that the specific needs of young carers are addressed, — to advocate for young carers’ rights individually and as a population, where there isn’t a team around the child or young person, — to support young carers and their families to ensure that children and young people will be protected from inappropriate caring and have the support they need to learn, develop and continue to enjoy positive childhoods and improved outcomes, — provide on-going identification and eradication of all inappropriate caring responsibilities for children and young people in Blackburn with Darwen.
