Support Service to enable adults who have had a stroke to achieve their goals

Swindon Borough Council is looking to commission a Stroke Support Service to enable adults who have had a stroke to achieve their goals The contract deliverables are as follows- • Enable service users to access activities, hobbies and information that they may have participated in before their stroke. • Introducing new activities that will provide communication opportunities. • Enabling individuals to gain confidence and independence post stroke. • Sign post to Carers support through the Swindon Carers Centre. • Refer employers and service users that are in employment to access to work a government scheme that provides practical advice and support for people and their employers to allow them to find or stay in work. • Support service users to access peer support through groups or Communication Cafes. • Work with appropriate live well managers to promote physical activity. As we are looking for innovation to fulfill this contract the Commissioner will work with successful organisations to set appropriate KPIs.
