Provision of Care and Support at Home - Short Term Reablement Services in Somerset

<p>Somerset County Council is intending to establish a Light Open Framework Agreement under which to procure Regulated Short Term Reablement services via the Proactis Portal -https://supplierlive.proactisp2p.com/Account/Login. The total value of the Framework over the agreement period for all services is estimated to be £4,000,000. We are seeking interest from organisations that would like to join the Framework and provide these services in Somerset. Somerset County Council is continuing its ambition, alongside our very successful Home First Hospital Discharge Service, to enable health services and social care to work closer together. Our focus is very much on reducing overall system pressures and to do this we need to continue to innovate and provide more services in the community rather than in hospital-based services. The vision for Health and Social Care in Somerset is focused on encouraging people to stay well through a focus on healthy lifestyle choices and self-care.
