Care and Support at Home Service

Tender description

The service will cover all client groups including but not limited to: Adults over and under 65, personal and social care and support with non-complex primary support needs of: • Dementia • Mental Health Conditions • Physical Disabilities • Learning Disabilities • Sensory Impairments • Autism • Substance misuse aged 18 or over. The Care & Support at Home Service must include: • Working in partnership with health and social care partners to provide services to people with a range of support needs, which includes (but is not limited to), Caring for Adults of 65 Years; • Caring for Adults Under 65 Years • Caring for people with Dementia • Caring for people with challenging behaviours living at home and to provide support to their families; • Caring for people with learning disabilities; • Caring for people with Autism • Caring for people with cognitive impairments; • Caring for people with frailty. • Caring for people with complex conditions • Caring for people with complex family dynamics The Care & Support at Home Service will deliver a reformed model of care and support in the community, delivering early intervention and preventative support through to intensive health and social care interventions. The service will support people to sustain and improve the quality of their life living at home, preventing deterioration and social isolation through regular monitoring and support, diverting people away from inappropriate and long term reliance on health and social care services unless they are absolutely necessary Keywords: Domiciliary Care, Care and Support, Re-Ablement, Complex Care, End of Life, Continuing Health Care
