Tender description
London and Quadrant is one of the largest charitable housing associations in the country and over the next 10 years will become one of the largest developers in the country, creating new homes and communities. Through the L and Q Foundation we continue to develop and fund social interventions that improve people’s chances in life and creates opportunities in our communities. The Independent Lives pillar of the L and Q Foundation provides a holistic, multi modular intervention service through in-house and commissioned support. However, the team has identified gaps in its current service provision and how the lack of a Health and Wellbeing (H&W) offer is impacting our ability to provide an all-encompassing offer to our residents. With that in mind, L and Q are looking to introducing an outreach service to our residents where by a diagnostic assessment can be completed either within the resident’s home or locally. With a person-centred approach, we are looking for an organisation to use its experience and skills to help encourage residents to take up opportunities either locally to them or via the provider themselves remotely. We are looking for a provider who has a track record in this type of delivery, understands this customer base, can provide a person-centred approach whilst encouraging residents to access services that will better their health. The initial programme will run for 24 months, engaging with at least 300 residents per year with at least 75 % engaging in some form of H&W initiative and at least half continuing to be engaged in the programme for at least 4 weeks. As part of our market testing we would like to speak to organisations, including consortiums, which can deliver this programme across South London, North and North-East London. Specific boroughs in which we would be looking to run a programme are: — Lewisham, — Southwark, — Haringey, — Hackney and Barking and Dagenham and Slough. These discussions will not constitute any form of commitment on behalf of L and Q or the provider(s) who take part.
