For the supply of patient-issued kits for the sampling of immunisable diseases. Public Health England, an executive agency of the Department of health and social care, require a supplier of disease surveillance kits. These kits are issued to healthcare providers and patients for the collection of clinical samples of various immunisable diseases. The supplier must be able to provide boxes, printed envelopes, instruction sheets and sample collection devices of the required specification, and must also be able to make up and hold stock of kits as required.
