Hertfordshire County Council (the “Council”) has undertaken a procurement for the provision of a Framework Agreement for Support at Home — Spot Providers in Saint Albans. This is an award notice and this tender opportunity is now closed. Please see Section V (Award of Contract) and Section IV.2.1 (previous publication concerning this procedure) for further details. Support at Home for Older People Spot Support at Home services in Saint Albans will be delivered through this Framework Agreement and are split in to Lots by care groups/sub-groups. Support at Home Providers are responsible for ensuring the delivery of Support at Home services for people over the age of 18 with Health and Social Care needs. This will predominantly be older people and those with a physical disability, but could include people with a Learning Disability and Mental Health Problems. The Services will cover seven days a week. Support at Home for people with a Physical Disability Spot Support at Home services in St Albans will be delivered through this Framework Agreement and are split in to Lots by care groups/sub-groups. Support at Home Providers are responsible for ensuring the delivery of Support at Home services for people over the age of 18 with Health and Social Care needs. This will predominantly be older people and those with a physical disability, but could include people with a Learning Disability and Mental Health Problems. The Services will cover seven days a week. Support at Home for people aged between 18 and 25 who have special educational needs (SEND) Spot Support at Home services in St Albans will be delivered through this Framework Agreement and are split in to Lots by care groups/sub-groups. Support at Home Providers are responsible for ensuring the delivery of Support at Home services for people over the age of 18 with Health and Social Care needs. This will predominantly be older people and those with a physical disability, but could include people with a Learning Disability and Mental Health Problems. The Services will cover seven days a week. Support at Home for people with Dementia, either older people, people with a physical disability or people with a learning disability Spot Support at Home services in St Albans will be delivered through this Framework Agreement and are split in to Lots by care groups/sub-groups. Support at Home Providers are responsible for ensuring the delivery of Support at Home services for people over the age of 18 with Health and Social Care needs. This will predominantly be older people and those with a physical disability, but could include people with a Learning Disability and Mental Health Problems. The Services will cover seven days a week. Support at Home for end of life service users who are either older, have a physical disability or have a learning disability Spot Support at Home services in St Albans will be delivered through this Framework Agreement and are split in to Lots by care groups/sub-groups. Support at Home Providers are responsible for ensuring the delivery of Support at Home services for people over the age of 18 with Health and Social Care needs. This will predominantly be older people and those with a physical disability, but could include people with a Learning Disability and Mental Health Problems. The Services will cover seven days a week. Support at Home for people with a substance misuse who are either older, have a physical disability or a learning disability Spot Support at Home services in St Albans will be delivered through this Framework Agreement and are split in to Lots by care groups/sub-groups. Support at Home Providers are responsible for ensuring the delivery of Support at Home services for people over the age of 18 with Health and Social Care needs. This will predominantly be older people and those with a physical disability, but could include people with a Learning Disability and Mental Health Problems. The Services will cover seven days a week. Support at home for people with sensory impairment who are either older, have a physical disability or a learning disability Spot Support at Home services in St Albans will be delivered through this Framework Agreement and are split in to Lots by care groups/sub-groups. Support at Home Providers are responsible for ensuring the delivery of Support at Home services for people over the age of 18 with Health and Social Care needs. This will predominantly be older people and those with a physical disability, but could include people with a Learning Disability and Mental Health Problems. The Services will cover seven days a week. Support at home for people with learning disabilities Spot Support at Home services in St Albans will be delivered through this Framework Agreement and are split in to Lots by care groups/sub-groups. Support at Home Providers are responsible for ensuring the delivery of Support at Home services for people over the age of 18 with Health and Social Care needs. This will predominantly be older people and those with a physical disability, but could include people with a Learning Disability and Mental Health Problems. The Services will cover seven days a week. Support at home for people with mental health problems who are either older, have a physical disability or a learning disability Spot Support at Home services in St Albans will be delivered through this Framework Agreement and are split in to Lots by care groups/sub-groups. Support at Home Providers are responsible for ensuring the delivery of Support at Home services for people over the age of 18 with Health and Social Care needs. This will predominantly be older people and those with a physical disability, but could include people with a Learning Disability and Mental Health Problems. The Services will cover seven days a week. Support at home for Live in Care for either older people, people with a physical disability or people with a learning disability Spot Support at Home services in St Albans will be delivered through this Framework Agreement and are split in to Lots by care groups/sub-groups. Support at Home Providers are responsible for ensuring the delivery of Support at Home services for people over the age of 18 with Health and Social Care needs. This will predominantly be older people and those with a physical disability, but could include people with a Learning Disability and Mental Health Problems. The Services will cover seven days a week. Support at home for rapid response for either older people, people with a physical disability or people with a learning disability Spot Support at Home services in St Albans will be delivered through this Framework Agreement and are split in to Lots by care groups/sub-groups. Support at Home Providers are responsible for ensuring the delivery of Support at Home services for people over the age of 18 with Health and Social Care needs. This will predominantly be older people and those with a physical disability, but could include people with a Learning Disability and Mental Health Problems. The Services will cover seven days a week. Support at home for carer support either for older people, people with a physical disability or people with a learning disability Spot Support at Home services in St Albans will be delivered through this Framework Agreement and are split in to Lots by care groups/sub-groups. Support at Home Providers are responsible for ensuring the delivery of Support at Home services for people over the age of 18 with Health and Social Care needs. This will predominantly be older people and those with a physical disability, but could include people with a Learning Disability and Mental Health Problems. The Services will cover seven days a week.
