Enhanced Primary Care Support for Nursing Homes

Tender description

In respect of this invitation to tender the objective of the exercise will be to commission an enhanced primary care support service for nursing homes in Walsall with dedicated cover delivering proactive care, however, each respective GP will retain overall responsibility for the patient. Tenders or requests to participate must be submitted electronically via: https://www.nhssourcing.co.uk Nursing homes may be contracted throughout the year to provide additional beds for specialised care for patients on a short term basis (normally 2 - 6 week). If the nursing home has contracts for these specialised care beds then they will also be included in the enhanced service provision. The additional beds may include, but not be limited to the following: Diversionary beds Step up / Step down beds ICT / Re-enablement / Palliative Care beds Continuous Healthcare beds Discharge to Assess / Treat beds.
