Tender description

Brain injury affects the way a person thinks, learns and remembers. Different mental abilities are located in different parts of the brain, so a head injury can damage some, but not necessarily all, skills such as speed of thought, memory, understanding, and concentration, solving problems and using language. Communication problems after brain injury are very common. Everyone who has had a head injury can be left with some changes in emotional reaction and behaviour. Executive dysfunction is a term for the range of cognitive, emotional and behavioural difficulties which often occur after injury to the frontal lobes of the brain. Impairment of executive functions is common after acquired brain injury and has a profound effect on many aspects of everyday life. The service will provide a day centre as a base to- • Help service user set and achieve a series of measured goals. • Where appropriate support service users to move on from the service to more independent community based services. • Provide cognitive therapy for service users. • Work with service users to help them to reach their potential after a brain injury. • Liaise with and utilise other organisations and networks to improve outcomes for the service user. • Ensure activities and goals are aligned with outcomes identified within care and support plans. • Ensure information about the service is up to date on the local directory of services (My Care My Support). • Complete quarterly monitoring returns. • The successful provider will be required to attend provider meetings organised by the Commissioner. • Actively involve service users in the design of service delivery. • The successful bidder will have to track service user's progress against agreed goals in the short, medium and long term. • The successful provider will be required to work closely with Healthwatch. • Develop and maintain referral routes with (including but not limited to) GPs, Occupational Therapists, Hospital, Social Workers and Adult Mental Health Services. • Work with relevant clinical team (s)
