Countywide Home from Hospital Service.

Tender description

North Yorkshire County Council currently commissions a county wide Home from Hospital service which supports people who do not have an eligible need but do require additional practical support to be safely discharged from hospital and established at home. The main types of support include (but not limited to): • provision of a food parcel on the day of discharge for those with no-one to shop for them; • assisting the person with shopping; • prescription collection; • dog walking or making arrangements for pets to be re-homed or temporarily cared for; • arrange for furniture to be moved within the house; • some light cleaning; • changing light bulbs; • negotiating with services such as utilities; or • escort to hospital, doctor, the dentist. This service currently delivers support mostly to older adults in North Yorkshire and helps to support the Council’s approach to delivering its responsibilities under the Care Act (2014) to promote wellbeing and prevent, reduce and delay the need for statutory social care services. It also supports delivery of the following key outcome in the North Yorkshire Health and Wellbeing Strategy 2015-2020: ‘People can make choices to self-manage their care to help them stay independent for longer.’ The current contract is due to end in March 2018, and the Council is required to re-procure service provision.
